STEP 1 - Complete the CID & Badge Identification Request Form:

STEP 2 - Some people will need to complete Dust Buster training.  Speak with your contact at McCarthy to confirm.

STEP 3 - Give or send (via email) your completed CID & Badge Form and your Dust Buster Training Certification of Completion to Rowena del Mar at Scripps Health:
